Please agree to the following as a client:
- To participate fully in sessions and audio sessions and to be responsible for your choices.
- To seek medical advice where needed and communicate with relevant professionals.
- The therapist has fully explained the process and procedures.
- Each session including the Initial Consultation costs £75 (or £150 for Smoking Cessation). Please try to give at least 24 hours notice if you cannot attend a session.
- For online work, please organise a suitable space and broadband connection.
If you are visiting in person, please confirm that for each visit:
- During the past two weeks, you have not had symptoms associated with the Covid-19 virus and you have not been in contact with anyone with or suspected to have COVID-19. (If you are feeling unwell or have had any symptoms please let us know. If necessary we can re-arrange your session.)
- You are not considered to be in the category of individuals at high risk or required to self isolate.
- You will inform us if any of these conditions change.
- We may need to share your name and contact details with NHS Track and trace if you, I or another client are tested and found to have the virus.

- Respect confidentiality (unless we believe a client or other may be harmed if information is not disclosed; or for NHS Track and Trace).
- Maintain high standards of personal conduct and safety.
- Maintain COVID-19 guidelines and adhere to our own risk assessment, which is available on request.
- Keep professional knowledge and skills up to date and engage in regular supervision.
Privacy
- If you visit the practice in person your contact details may be made available to NHS Test and Trace if someone who has tested positive for COVID-19 lists our practice as a place they visited recently.
- We will only use your information to provide the agreed services. You can view or change your details at any time. Read our full privacy policy here.
